Office visits are scheduled by our receptionists. Scheduling of surgery is done by the office manager.
Except in the case of emergencies, it is not our policy to see patients without an appointment. Our schedule is often filled well in advance, and we do everything we can to assist you if delays or postponements occur. If you leave us a business or home phone number on the day of your appointment, we can call you so that you might adjust your schedule accordingly. Often Dr. Wade is called to the hospital, or some patients require extra clinic time for their serious injuries. We appreciate your understanding and patience with us as we do the best we can in managing our time constraints.
If your time is short, we encourage you to call ahead and check our clinic schedule time. We will be glad to accommodate you at a later time that day.
If you need to request medical records from our office, please note that it may take 7 working days to fill your request. We will get your records to you as quickly as possible, and the more time you allow will ensure your success in getting them in the time span you desire.
ACCEPTED INSURANCE CARRIERS
For patients whose injury is work related, you must provide the clinic with an authorization letter from your employer or agent. Please include a claim or case number, contact agent and their phone number, and appropriate billing address.

WHAT TO BRING WITH YOU TO YOUR CLINIC APPOINTMENT
Your insurance card and a photo ID. Some insurance carriers require you to have a referral or primary care physician authorization before treatment is given. Please check with your carrier if you are in doubt.
Athletic shorts (hip and knee examinations only.) Clinic notes and operative reports from any previous surgeon or physician you have seen relating to your injury. Xrays or MRJ's not older than 12 months old.
Payment in full is expected at the time of your visit. Exceptions include workers compensation, Medicare, and some copay agreements. Check your insurance information to make sure what is expected of you before you arrive. For your convenience, we accept Visa, MasterCard, cash or private checks. Please contact our office manager if you have questions about payment for your visit.
Like most physicians' offices, we request payment at time of service unless previous agreement has been made. Since most insurance companies do not reimburse the total amount due, you will be responsible for any charges that are not covered under your plan, and any co-payment due after your appointment. We recommend reviewing your insurance plan to become familiar to what your responsibilities will be.
REHABILITATION AND RECOVERY
If your injury requires surgery or physical therapy, we understand the importance of rehabilitation to your treatment. Often customized rehabilitation programs are designed to meet your specific need. Dr. Wade works with most physical therapy clinics in the area, and works with you to fit your transportation and injury requirements.
